New Hire Informaton

In order to begin the New Hire process, you will need to complete the New Hire Forms listed below. If you have any questions or would like to make an appointment to submit your paperwork, please email the HR Office at hr@law.ucla.edu. The HR Office will provide electronic versions of these forms for completion via DocuSign.

New Hire Paperwork Instructions:

  1. Review all the required forms listed below.
  2. Complete the forms in advance, leaving all dates and signatures blank. We will assist you with these fields when you arrive in our office (55 Dodd Hall) for your scheduled appointment. If completing this information remotely, contact HR@law.ucla.edu for specific instructions.
  3. Immigration laws require that you provide original and official forms of identification; copies and/or faxes will not be accepted. A list of eligible documents is listed on Page 4 of the I-9 Form (provided below for your reference only - do not complete). You may also find a list of acceptable document(s) to present here: https://www.uscis.gov/i-9-central/form-i-9-acceptable-documents.

    All new hires will receive a link to an electronic I-9 through a system called Tracker (employment.authorization@universityofcalifornia.edu). New hires will be prompted to complete section 1 no later than their first day of work. 

           For example, the most common combination of documents we receive include:​

  • ​Driver's license AND Social Security card

Or

  • US passport (your passport AND Current Visa if not a U.S. citizen along with your F1 or J1)

Please note that you may NOT begin working until you have submitted these forms along with the official and eligible forms of identification. No exceptions will or can be granted.

New Hire Forms:

  1. Application Form (Required only for Student, NGOP, and Limited or Short-Term Appointments)
  2. State Oath (Required for all new hires)
  3. I-9 Form (Provided only for your reference for page 3)
  4. Mailed Paycheck Acknowledgement Form (Required for all new hires)
  5. W-4 Tax Information (Provided only for your reference)
  6. Demographic Data Form (Recommended)
  7. Voluntary Self-Identification of Disability Form (Recommended)
  8. Statement Concerning Social Security (Required only for Student, NGOP, and Limited or Short-Term Appointments
  9. Emergency Contact Information Form (Required for all new hires)
